The clerk step

Why a Health copy needs a $12 certificate.

The Secretary of State authenticates the signatures of California public officials. For a birth or death certificate signed by a health officer or local registrar, its guidance says county clerk certification “may be required.”

San Mateo County settles it in writing: “Apostille requests accompanied by a Health Department issued birth or death certificates (dated 1966 to the present) also require a Public Health Officer’s Signature Verification certificate issued by the County Clerk-Recorder Division. The cost is $12.” A Clerk-Recorder copy, birth, death or marriage, “can be submitted directly to Secretary of State’s Office without additional documentation.”

The verification is issued by the Clerk-Recorder in Redwood City, not by Health in San Mateo. We obtain it, attach it to the certificate, then file in Sacramento.

Rejection traps

What sends San Mateo County files back.

We screen for each one in the $45 Document Check.

01A Health copy sent straight to Sacramento. Without the $12 Public Health Officer’s Signature Verification certificate from the Clerk-Recorder, a San Mateo County Health birth or death certificate is not ready for the Secretary of State.
02A fee from an old form. The county’s marriage record application still lists $17, while the current fee schedule shows $19. A short payment stalls a mail order.
03A pre-1966 record on a tight deadline. The Clerk-Recorder stores births before 1966 off-site and needs 24 hours’ notice, so a same-day pickup is not possible for older records.
04A photocopy instead of a certified copy. One of the most common catches in our Document Checks. A photocopy carries no official’s signature, so it cannot be apostilled. Fix: a new certified copy from the issuing office.

02

Where do I get the Public Health Officer’s Signature Verification?

From the Assessor-County Clerk-Recorder, 555 County Center, 1st Floor, Redwood City. Health issues the certificate; the Clerk-Recorder issues the verification. The county fee is $12.

From San Mateo Superior Court Records Management, Hall of Justice, 400 County Center, 1st Floor, Room A, Redwood City.
